Root-Cause Analyses. This moves the system from simply "counting
accidents" to "understanding and fixing" the underlying system failure.

For years, patient safety incidents in Ethiopia were under-reported due to the burden of paper forms and a fear of professional repercussions. Fenot partnered with the HSIQ-LEO to launch the e-PSIRLS. In our pilot phase, we saw a measurable increase in the quality of reported near-misses. By moving the system to a secure, digital platform, we have empowered healthcare providers to speak up. Today, what was once a hidden 'incident' is now a digital data point driving national policy changes and preventing preventable harm for 4 out of 10 patients across the country.
